Book an Occupational Therapy consultation session

WHY WOULD YOU SCHEDULE A CONSULTATION?
If you are interested in working with me, choose a time for an initial consultation and we can spend the hour discussing your needs. The 1 hour consultation is billed at a regular session rate of $200.  This could be a one-time consultation, if that's what you're looking for. BEFORE you click the button below, please read the following:


WHAT YOU ARE AGREEING TO:
In signing up for a consultation, you are authorizing me to:
1. Collect your personal (name, phone number, email) information to book the time using a NON-Canadian scheduler/server and sending me an email notification through a NON-Canadian email platform/server.
2. Collect and document the personal and health information you share with me during our video call on an electronic medical record which is PIPA and PIPEDA compliant.
3. Keep all of this information PRIVATE and CONFIDENTIAL and NOT share it with anyone else, UNLESS: you disclose a risk of harm to yourself or others; you share information that I am legally required to disclose; or this record is subpoenaed.

PLEASE NOTE that, if this consultation is for services for a minor child (anyone under 18 years of age), the consultation will be with the PARENT/GUARDIAN.


WHAT I CAN OFFER YOU:
In choosing to book a video consultation with me, you understand that the purpose of this consultation is to help you understand what I can do for you (or your child) and that I will be providing you with immediately relevant suggestions based on the information we discuss regarding your current situation.  This may include recommending assessments or consultations with other medical or paramedical professionals.

If, based on the discussion during this consultation, I feel further interventions (consultations, assessments, education, or therapy) is warranted or could be helpful to you (or your child), I will advise you of how we may be able to work together, including potential risks & benefits to the occupational therapy services I can offer.

If I do NOT not think that I can be of further help and you have additional needs, I will provide suggestions for you to find services to better meet your needs (e.g. through other agencies or service providers like Alberta Health Services, community agencies, other providers, etc.).

The pertinent details of this conversation will be documented in an electronic health record.
